Specialty Care

Condition-specific support for Alzheimer’s, diabetes, Parkinson’s, hospice, and the other diagnoses that change what good care looks like.

Care built around a diagnosis

A diagnosis changes the details. Someone with mid-stage dementia needs a caregiver trained in redirection and routine; someone recovering from a stroke needs one trained in transfer safety and aphasia-friendly communication. Specialty care means the caregiver arrives already trained for the condition in front of them.

Conditions we support

Each condition below has its own page describing what care typically involves, what families should watch for, and how a plan is usually structured.

  • Alzheimer’s, cognitive, and dementia care
  • Parkinson’s disease
  • Diabetes care
  • Chronic condition care, including COPD and heart failure
  • Hospital-to-home transitions
  • Hospice and palliative support
  • Adults with disabilities
  • Veterans care

Training that goes beyond the minimum

Caregivers complete condition-specific coursework through our caregiver training program before being assigned to a specialty client, and continue training annually. Where a condition is progressive, the plan is reviewed on a set schedule rather than only when something goes wrong.
