For discharge planners and case managers
We take referrals directly and can usually have care in place within 24 to 48 hours, including same-day for urgent discharges. We provide documentation of the care plan and visit compliance for your records.
Reducing readmissions
Transitional care is designed around the causes that drive avoidable readmissions: medication errors, missed follow-ups, poor nutrition, and unsafe home environments. We can report on the specific interventions delivered post-discharge.
Working with hospice and palliative teams
Our caregivers supplement rather than duplicate the hospice team, covering the hours between clinical visits with personal care, meals, comfort measures, and family respite.
Community organizations
We partner with Area Agencies on Aging, senior centers, faith communities, and disease-specific associations on education, support groups, and resource referral.
