Chronic Condition Care
Long-term support for COPD, heart failure, arthritis, stroke recovery, and the other conditions people live with for years.
Living with it, not recovering from it
Chronic conditions do not resolve. Care has to be sustainable over years, which means it has to fit into a real life rather than dominate one. Plans are deliberately conservative at the start and scale up as needs change.
Conditions commonly supported
- COPD and other chronic respiratory disease
- Congestive heart failure
- Stroke recovery
- Arthritis and chronic pain
- Kidney disease and dialysis support
- Multiple sclerosis and ALS
- Cancer during and after treatment
Catching decompensation early
For conditions like heart failure and COPD, the difference between a phone call and an ambulance is usually a few days of noticing. Caregivers are trained on the specific warning signs for the client's condition, weight gain, swelling, increased breathlessness, and on who to call when they see them.
Talk to someone who knows this condition
Our care coordinators can walk you through what care usually looks like at this stage, what it costs, and what your options are.
